We've had a health savings account (HSA) for 2 1/2 years now. When we opened it there weren't many options available and so I braced myself for the fees. But they never came. I've received several updated "fee schedules" but never have there been any deductions for fees from the account.
Today we received a notice that our service fees would "no longer be paid by a third party on your behalf." Hmmm.... Where is this third party? We would like to say thank you. Actually, I have a few other things we wonder if you would be interested in paying?
Fortunately, there are MANY options for HSAs now and we already have a second account with no fees in place. So it is just a matter of closing the account. But I suspect that the mysterious "third party" is just the person on the bank end who opened the account and checked the wrong box (suppress fees) and all those who never caught it thereafter. But a "third party" does sound much nicer.

I've been very happy with our HSA account. Not only is it free, but they also pay 5.00% interest--better than I've been able to find anywhere. They're located out of Little Rock, Arkansas, but as long as you don't mind sending a check in the mail to make the initial deposit, you can otherwise do all of your other transactions (including subsequent deposits) online. It's Delta Trust & Bank.
